Our NDIS services includes Community Care, Nursing & Complex/High Intensity Supports, Respite Care/Short Term Accommodation (STA), Supported Independent Living (SIL), Domestic Assistance (Assistance with Household tasks, Personal activities, etc)

Steps:
Passion Aid Care Services employs a systematic approach in creating, implementing, and refining support services that are fundamental in establishing a successful and person-centric NDIS support operation.

Intake meetings
At the beginning, we will hold intake meetings with all relevant individuals, stakeholders, and the Care Team, if applicable. These discussions assist in identifying the needs and expectations of the service user.

Staffing ratios
Working together with the Care Team to determine suitable staffing ratios, levels, and capacities, to ensure efficient fulfillment of the requirements of each supported individual.

Workforce
It is important to us that you are supported by the right staff with the right competencies and skills needed to perform the shift requirements and deliver the quality service that we promise. We promote continuous learning through regular in-house and external trainings.

Support Plans
Our team collaborates closely with all involved parties to develop customized support plans, risk evaluations, alerts, and goals. These thorough plans serve as the basis for providing safe and effective support services that focuses on the supported individual.

Transition or Implementation
We take great care in managing the implementation of your plans as well as the process of transitioning to Passion Aid Care Services, ensuring that it is a smooth and seamless experience for our participants as they commence our care and support services.
